Name, email, phone, and your U.S. state of residence. Our affiliated physician network is licensed across all 50 states.
Current medical info
Organize any major medical conditions and current medications. Be honest — the physician uses this to confirm the program is safe for you.
Your symptoms
A short, honest read on what feels off. Energy, drive, recovery, sleep, mood. No need to be clinical — plain language is fine.
Five minutes of focus
The process is short but the questions are important. Plan for five minutes when you won’t be interrupted. Shortly after submitting your application and based on your preference, a member of our team will either coordinate a time for a brief phone call or send you a digital questionnaire.

What happens next: a care coordinator reads your submission within 24 hours and will contact you via your preferred method to complete our questionnaire. If you’re a fit, you’ll be invited to schedule the intake consult — your first charge happens after you confirm. If you’re not a fit, we’ll tell you honestly — before any payment.
What Happens, and When.
Within 24 hours
A care coordinator will contact you to guide you through the next steps. After you've completed our questionnaire, you'll hear back within 24 hours by email or phone — whether accepted or not. Declines come with a brief, honest explanation.
Within 3 business days
If accepted, you’re scheduled for a non-clinical onboarding call and issued a lab order for a LabCorp or Quest near you. No charges until the onboarding call.
Within 7–10 days
Baseline labs complete, physician consult done, protocol designed, custom-compounded medication shipped to your door in unmarked packaging. You’re on.
